Doctor/Healthcare professional's consent form - required before undertaking any of the therapies provided by the centre
Infrared Sauna GP Form - this form provides the information from your GP or Consultant that they are aware of your wish to commence Infrared sauna therapy
Gift Aid Declaration form - so that we can claim 25% of your donation back from the tax man if you pay income tax or capital gains tax equal to the tax we reclaim on your donations.
Download our Sponsorship Form for your event or if you're collecting from friends and family. Don't forget to tick the Gift Aid box if the individual donation is from a tax payer - it all helps!
